Student Health Packet: Immunization & Health Forms

In the interest of maintaining a safe and healthy campus, all full and part-time students, new to NJIT, are required to complete the Pre-Entrance Health Requirements packet.

Student Health Packet

New first-time students, including full- and part-time, international, undergraduate, graduate, and doctoral programs, must submit this form.

All forms in the Student Health Packet are to be submitted through the Medicat Patient Portal at njit.medicatconnect.com.

Please read the requirements carefully to make sure you are in compliance.

Immunization Documentation Guidelines

Acceptable Immunization Forms

NJIT Immunization Record Form (attached); or

An official immunization record from a healthcare provider (signed, dated, and stamped); or

Record from a previous school, state immunization database, military branch, or employer; or

Lab report showing immunity (the actual laboratory results must be submitted for review and acceptance)

Accessing Your Immunization Records

Individuals immunized in New Jersey (as well as Alaska, Idaho, Maine, Minnesota, Missouri, North Dakota, Utah, and Wyoming) can download a PDF copy of their immunization records through the Docket app.

Individuals immunized in other states can access their records through their state’s immunization database.

Invalid Documentation

Documents that have been modified (e.g., overwritten, crossed out, or whited out) are not considered valid.

Documents without an official English translation are also not considered valid.

Translation Requirement for Immunization Records

If your immunization records are not in English, they must be professionally translated and include a notarized stamp or seal before they can be accepted. Records submitted without proper translation or notarization will not be processed.

Low-Cost Immunization Services (Vaccines & Physicals)

If you need vaccines or a physical exam and are looking for affordable options, the following programs and clinics may be helpful:

Vaccines for Children (VFC) Program

For individuals under 19 years old who are:

Medicaid-eligible

Uninsured or underinsured

*Under 18 and accompanied by a parent or guardian

To participate, you must first schedule a physical exam with a VFC provider before receiving vaccines.

317-Funded Vaccine Providers

For individuals 19 years of age and older who:

Do not have health insurance

Are underinsured (insurance does not cover some or all recommended vaccines)

Before You Go:
Some locations may only vaccinate their established patients or require an appointment. Please call ahead to confirm availability and eligibility.
